How to Refer a Client

Referring a client to DNM Health Services is straightforward. Providers may submit referrals through multiple channels based on preference and urgency:

To help us process referrals efficiently, please include the client’s name, date of birth, contact information, reason for referral, and any relevant clinical information. What Happens After a Referral is Submitted

Our team works promptly to ensure a smooth transition of care. Once a referral is received, we will contact the client within 24–48 business hours to begin the intake process. This includes verifying insurance, assessing scheduling availability, and matching the client with a clinician who aligns with their clinical needs and preferences.

With proper authorization, we are happy to collaborate with referring providers and provide updates as needed to support coordinated care.
