We make the referral process simple, efficient, and responsive so that individuals can quickly access the mental health support they need. Whether you are a medical provider, therapist, school professional, or community organization, we welcome the opportunity to support your clients with high-quality, compassionate care. We provide outpatient mental health counseling services for individuals, couples, and families.
Our clinical team offers support for a range of concerns including anxiety, mood disorders, trauma, ADHD, and life transitions. If a client’s needs fall outside of our scope, we work closely with trusted referral partners to ensure continuity of care.

How to Refer a Client

Referring a client to DNM Health Services is straightforward. Providers may submit referrals through multiple channels based on preference and urgency:

  • Online Referral Form: Submit a secure referral through our website
  • Fax: Send referrals to 678-666-3850
  • Phone: Call our office at 470-410-7738

To help us process referrals efficiently, please include the client’s name, date of birth, contact information, reason for referral, and any relevant clinical information. Insurance details are also helpful when available. Please ensure that appropriate consent has been obtained prior to sharing any protected health information.

What Happens After a Referral is Submitted

Our team works promptly to ensure a smooth transition of care. Once a referral is received, we will contact the client within 24–48 business hours to begin the intake process. This includes verifying insurance, assessing scheduling availability, and matching the client with a clinician who aligns with their clinical needs and preferences.

With proper authorization, we are happy to collaborate with referring providers and provide updates as needed to support coordinated care.
